Registration Information
We’re happy to have you join us. Please remember you must read the Parent Handbook and return fully completed registration forms prior to your child’s first reserved day. You will need to pay the registration fee and purchase your first block of hours on your child’s first day. Your medical forms(Health Care Summary and Immunization form) MUST be completed by your doctor’s office and be received by KidsPark before you can reserve time for your child’s first day. All forms may be faxed to KidsPark at 651-642-0724.
Note: The Minnesota Department of Health provides the Immunization Form and it states that for children older than 15 months a health care provider signature is not required. However, KidsPark licensing and KidsPark policy requires a health care provider signature for all children, including those older than 15 months.
Returning families must re-register by submitting the yearly membership fee along with new contracts, parent information forms, and emergency cards. New health care forms and immunization records must be completed by your health care provider for each returning child.
